that access to which is to be disabled. You should include the specific
URL or URLs of the web pages where the allegedly infringing material

A statement that the you have a good faith belief that use of the material
in the manner complained of is not authorized by the Copyright Owner, its
agent, or the law.
A statement that the information in the notification is accurate, and
under penalty of perjury, that you are the owner or authorized to act
on behalf of the Copyright Owner of an exclusive right that is allegedly
infringed.
If you are a copyright owner or an agent
thereof and believe that any user submission or other content
infringes upon your rights, although we are not governed by it,
you may submit a notification pursuant to the Digital Millennium
Copyright Act ("DMCA") by providing Ucaster with the
following information in electronic form (see 17 U.S.C. 512 (3)
for further details):

Upon reception of proper notification we will confirm the existence of
material, notify the registered user who posted the content.
It is our policy to remove or disable access to infringing content, notify
user that his content is removed or disabled and repeated infringements will
lead to the termination of account.
